Add.Me

The Add.Me is a secure web form that enables clients to add themselves to our database for the purpose of receiving counselling or group support. It is important to complete each field with an asterisk beside it. In the case where there is a drop-down menu, you will be able to select the heading which has the most appropriate support pathway. If there are mandatory fields on the form (marked by an*) that you do not know, or do not have, you can type in [email protected] or "no fixed address" for example.

By completing this form, you will be added to ACGB's waiting list to receive grief and bereavement counselling/group support. Following the submission of this form, you will be contacted by an ACGB staff member as soon as possible within business hours. No referral is needed, however the individual receiving the support needs to provide verbal consent and complete a consent form and questionnaire.
